What You’ll Do
Overview
The Market Clinical Pharmacist as part of the multidisciplinary care team provides direct patient care services for pediatric patients with chronic medical conditions focused on optimizing medication therapy, disease outcomes, and value of care. Working with the market clinical team the Clinical Pharmacist takes ownership of the medication management though participation in innovative patient visits including transitions of care, patient education, comprehensive medication management, disease state management, and shared visits. The Market Clinical Pharmacist provide direct oversite and support to market-based Clinical Pharmacy Technicians
- Practices pharmacy in compliance with all governing regulatory authorities
- Provides direct oversight of Market Clinical Pharmacy Technicians through review of medication histories, prior authorizations, prepared renewals, and support as requested by the clinical pharmacy technician.
- Provides pharmaceutical care services through direct interactions with patients/caregivers, internal and external care team members, and works independently under individualized scope of clinical practice
- Assists in care coordination for medication related issues between the patient, pharmacy, insurance, and medical provider through oversite and support of the market Clinical Pharmacy Technician
- Review patient medication profiles and apply evidence-based medicine to ensure appropriate drug selection and dosing scheduled and develops recommendations made to internal and external care team members for optimizing the patient's medication regimen
- Provides accurate, thorough medication education to patients/caregivers on drug interactions, side effects, dosage, and storage of medications
- Assesses medication adherence and barriers to adherence, intervenes to optimize medication adherence
- Provide medication education, comprehensive medication management, and disease state management services through virtual or face-to-face visits
- Obtains comprehensive medical and/or medication histories from patients and/or their records and summarizes the significant findings as documented in the medical record to ensure safe and effective care
- Designs, implements, assesses, monitors, and documents therapeutic plans utilizing the most effective, least toxic and most economical medications under scope of practice
- Initiates, modifies, renews, refills, or discontinues medication therapy using approved delegation procedures or in accordance with collaborative practice agreements
- Documents encounters and interventions in the patient electronic medical record, including closed-loop communication with patient care team
- Follow-up with patients/caregivers and internal and external care team members to support progress towards treatment goals
- Documents/reports adverse drug events (ADEs), medication allergies and medication errors per procedure
- Serve as a drug information resource to the multidisciplinary team
- Participates in market quality improvement initiatives to enhance and ensure patient safety for all patients.
- Works with Director and Clinical Pharmacy Specialists to identify, create, or edit policies and procedures affecting areas of responsibility.
- Conducts population-based medication utilization evaluations (MUE) to identify areas for medication use process optimization as requested by Director.
- Promotes the practice of evidence-based, cost-effective medicine by keeping abreast of new drugs, devices, and medical literature and through support of use of the Imagine Pediatrics Formulary
- Demonstrates the ability to set priorities, work independently and cooperatively with others within and outside the department.
- Presents information (verbal and/or written) in a clear and concise manner.
- Displays friendliness, compassion, sensitivity, kindness and appropriate manners in interactions with patients, physicians and other ancillary healthcare staff.
- Displays adaptability by adjusting to meet the needs of the patient and organization.
- Incorporates constructive feedback in a positive manner for improvement and necessary changes in, but not limited to, scheduling, procedures and assignments.
- Exhibits behaviors, which demonstrate competence, reliability, self-control, honesty, and appropriate overall appearance while rendering service.
- Performs all other duties as assigned by appropriate management.
- Demonstrates initiative when opportunities are presented as evidenced by successful completion of projects.
- Acts as a liaison between the pharmacy, nurses, physicians, and patients.
What You Bring & How You Qualify

- Graduate of an accredited college/university of pharmacy (Pharm.D) required
- Valid license or eligible for licensure from the Board of Pharmacy in the state(s) of associated market
- Completion of ASHP accredited PGY-1 Pharmacy Residency required, preferred settings include pediatric or ambulatory care
- Completion of an ASHP accredited PGY-2 Pediatric or Ambulatory Care Pharmacy Residency preferred
- Board Certified Pediatric Pharmacy Specialist (BCPPS), Board Certified Pharmacotherapy Specialist (BCPS), OR Board Certified Ambulatory Care Pharmacists (BCACP), preferred
- Bilingual English/Spanish preferred but not required
- Completion of a Pharmacy-Based Immunization Delivery Certificate program, preferred
Work Experience
- 2+ years of practice experience, post-PGY1 residency, in a pediatric or ambulatory care setting, preferred
