Overview

Mon - Fri, no weekends/evenings. Manage chronic conditions of patients on your own panel, seeing 6-8 patients per day.

Responsibilities

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

  • Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
  • Federal DEA License/eligibility
  • Geriatric training/experience preferred
  • Active CPR/BLS Certification
  • Board Certification

Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
