What are the job responsibilities?
This position is a dual position.
Social Worker Position Overview: We are seeking an experienced and dedicated Licensed Hospice Social Worker to join our interdisciplinary team and provide comprehensive psychosocial support to patients and families facing end-of-life issues. The ideal candidate will have a strong background in social work, counseling, and palliative care, with a deep understanding of the complexities of grief, loss, and bereavement. The candidate will be open to travel as necessary within the designated area, and any other duties as assigned by leadership.
Key responsibilities include:
- Comprehensive Psychosocial Assessment
- Counseling and Supportive Interventions
- Advance Care Planning and Decision Support
- Resource Referral and Coordination
- Bereavement Support and Follow-up
Bereavement Coordinator Position Overview: We are seeking an experienced and empathetic Hospice Bereavement Coordinator to join our interdisciplinary team and provide comprehensive bereavement support to families and caregivers following the death of a hospice patient. The ideal candidate will have a strong background in grief counseling, bereavement support, and community outreach, with a deep understanding of the complexities of grief and loss.
Key responsibilities include:
- Bereavement Assessment and Support
- Support Group Facilitation
- Individual Counseling and Follow-up
- Memorial Services and Remembrance Programs
- Community Outreach and Education
What do you need?
- Master’s degree in Social Work (MSW) from an accredited program and a licensure for the state of Indiana
- Minimum of one-year of experience in social work, grief counseling, bereavement support or related in a healthcare field, preferably in hospice or palliative care.
- Clean driving record, and valid driver’s license and auto insurance
