What are the job responsibilities?
Reporting to our Clinical Manager, the Nurse Practitioner is an extension of the physician responsible for gathering clinical findings to help determine continuing eligibility for hospice patients. The Nurse Practitioner covers PRN on-call emergency face-to-face visits with hospice patients and coordinates with the RN Case Manager, IDT Team, and sponsoring physician on all patient needs.

- Consultation and Education
- Symptom Management and Pain Control
- Care Coordination and Collaboration
- Documentation and Compliance
What do you need?
- Graduate of an accredited APN program and NP license for state of Ohio
- Clean driving record, and valid driver’s license and auto insurance
