Position Overview: Reporting to our Clinical Manager, the Hospice Care Assistant is responsible for providing exceptional care to patients in their final stages of life. This position involves providing direct care and support to hospice patients and their families, ensuring comfort, dignity, and respect throughout the end-of-life journey.

Key Responsibilities:

  • Patient Care and Support:
    • Provide hands-on assistance with activities of daily living (ADLs) for hospice patients, including bathing, dressing, grooming, and toileting, while preserving their privacy and dignity.
    • Assist patients with mobility, transfers, and positioning to promote comfort and prevent complications related to immobility.
    • Support patients with nutrition and hydration needs, including feeding assistance and monitoring intake and output as directed by the interdisciplinary team.
  • Comfort Measures and Symptom Management:
    • Assist in implementing comfort measures and non-pharmacological pain management interventions as directed by the hospice nurse or interdisciplinary team.
    • Monitor and report changes in patient condition, including pain, discomfort, or other symptoms, to the nurse as directed for appropriate intervention.
    • Provide emotional support and companionship to patients and their families, actively listening to their concerns and providing reassurance as needed.
  • Communication and Collaboration:
    • Maintain open and effective communication with the interdisciplinary team, including nurses, social workers, chaplains, and volunteers, to coordinate care and address patient needs.
    • Document care provided accurately and timely in the electronic health record (EHR) system, following established protocols and guidelines.
    • Collaborate with family members and caregivers as appropriate to ensure they are informed and involved in the patient's care plan and decision-making process.
  • Environmental and Safety Support:
    • Maintain a safe and clean environment for patients, including regular cleaning and disinfection of patient care areas and equipment.
    • Assist in ensuring a comfortable and supportive atmosphere for patients and families, including maintaining a peaceful and calming environment conducive to end-of-life care.
    • Follow infection control protocols and safety procedures to prevent the spread of infections and minimize risks to patients and staff.

Qualifications:

  • Current Certified Nursing Assistant (CNA) certification in state of practice.
  • Minimum of 1 year working as a CNA in hospice, palliative care, or a similar healthcare setting.
  • Current automobile insurance and valid driver's license required.
  • Strong understanding of end-of-life care principles, pain management, and symptom control.
  • Compassionate and empathetic demeanor with the ability to provide emotional support to patients and families facing end-of-life issues.
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary team members, patients, and families.
  • Ability to work independently and as part of a team, with a strong sense of responsibility and commitment to providing high-quality care.
  • Flexibility to adapt to changing patient needs and priorities, with the ability to prioritize tasks and manage time effectively in a fast-paced environment.
