Role Description

As the Atlas Oncology Partners Physician, you will provide direct clinical care to patients with cancer in close collaboration with our partner oncology practices. You will support and enhance the provision of high quality, evidence-based, whole-person care by diagnosing, evaluating and managing cancer-adjacent comorbidities as well as coordinating care and communicating with existing primary care and specialty providers. You will manage a longitudinal panel of patients who are actively engaged with our partner oncology practices to optimize the opportunity for patients to achieve maximum health outcomes throughout all phases of their cancer journey.

Reporting: This role will report to the VP of Clinical Operations

Responsibilities include, but not limited to, the following:

· Provide direct, evidence-based primary care to patients with cancer, both the office and virtually

· Maintain close collaboration with the partner oncology providers who are managing the patient’s cancer

· Coordinate care and optimize referrals to established primary care and specialty care providers in the community

· Participate in the delivery of specialized clinical programs, including congestive heart failure, frailty, and behavioral health

· Provide transitions of care visits

· Escalate clinical issues to the supervising Market Medical Director when appropriate

· Collaborate with VP of Clinical Operations and Practice Manager to continuously improve clinical operations

· Engage in achieving high scores on quality measures

· Participate in interdisciplinary care team activities, including huddles and case reviews, with other members of the Atlas Oncology Partners care team

· Perform other related responsibilities as needed

Required Qualifications

· You hold an unrestricted MD or DO state license, including board certification for Internal Medicine or Family Medicine valid in the state of practice

· At least 4 years of direct patient care experience in primary care

· Extensive knowledge and experience in areas relevant chronic disease management, and palliative care

· Strong ability to collaborate with diverse teams, build relationships, and foster a positive work environment

· Comfort with change, process improvement, and working in a start-up or growing organization

Preferred Qualifications

· Specific domain knowledge and/or direct clinical experience managing oncology and/or other complex, high acuity patients

· Proficiency in coordinating and collaborating with various healthcare providers and interdisciplinary teams to ensure seamless care transitions, effective communication, and integrated care for patients

· Excellent verbal and written communication skills to effectively interact with patients, families, healthcare providers, and other stakeholders

· Experience working in a rapidly growing, Value Based Care (VBC) environment
